The benefits of a happy, healthy smile are immeasurable! Our goal is to help you reach and maintain maximum oral health. The better we communicate, the better we can care for you.

Health History

When you arrive for your first visit, please be prepared with your health and medication information, which will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office, and us with you. If time permits, these forms will be sent to you in advance of your appointment, or you may download them by clicking on the link below. All forms require Adobe Acrobat Reader to view and print. Dental Insurance

Please bring your dental insurance card and any other dental insurance information to assist in your reimbursement. Please note that payment is due in full at the time of your appointment – we do not accept assignment from any insurance companies. As a courtesy, we will electronically file insurance claims for you, and you will be reimbursed directly by your insurance company based on the type of policy you have. Typical reimbursement time is approximately two-four weeks.

Patient Confidentiality

We believe maintaining patient confidentiality is a priority. Our office is HIPAA Compliant. We will request that you review the “Notice of Privacy Practices”, “Consent for Use and Disclosure of Health Information”, and “Acknowledgement of Receipt of Notice of Privacy Practices”. We will either mail this to you prior to your appointment, or you may download it as part of our New Patient Forms Package, and bring it with you to your first appointment. This can also be completed if you prefer to arrive early for your first appointment. These forms will need to be signed.

First Visit

At your initial exam we will obtain necessary digital x-rays, diagnostic models, and bite registration. Dr. Strumpf will perform an oral cancer evaluation and determine the status of your existing dentistry and periodontal status. He will discuss options for treatment and a "smile analysis" will be performed should cosmetics be an issue for you. Please bring photo ID, and if you have a full series of x-rays (16-20 films) which are current within 2 years at another dental office, we will be happy to request a copy. Please be sure to fill out the release form included in the New Patient Forms Package in order for us to make the request.

CONFIRMATION OF APPOINTMENTS: Depending on your preferences, we will either call you two business days prior to confirm your appointment, or you will receive an email and text reminder generated by our automated SmileReminder service. This reminder service sends you a message 2 weeks, 3 days and 2 hours prior to you appointment, but can personalized to better suit you.
 

CANCELLATION OF APPOINTMENTS: If you cannot keep an appointment, please try to inform us 48 hours in advance if possible. This courtesy will allow us to give that time to another patient.
